LiveBetter has an exciting opportunity for a Carer Support Planner to join our Carer Gateway team. As a Carer Support Planner, you will support carers to assess their needs and develop personalized support plans using tools like the Carers Star™. You will assist carers in identifying their strengths, informal networks, and community resources to help improve their well-being. Carer Support Planner also coordinate access to services, including emergency respite care, and help carers navigate external support systems such as My Aged Care or the NDIS. Additionally, they monitor and evaluate the effectiveness of support provided and collaborate with both internal and external teams to ensure carers' needs are met. This position is a fixed term part time position 15.2 hrs/week (Parental Leave Backfill for Approx.16 months) that will commence as soon as practicable. About You The successful candidate will possess: - Ability to pass a National Criminal History Check
- Background in Community Services, Health, Welfare, Social Services, or Education, or a minimum of three years’ relevant industry experience.
- Ability to conduct complex telephone assessments, gather accurate information, and prioritize actions effectively.
- Strong interpersonal skills with the ability to provide superior customer service and communicate effectively with diverse stakeholders.
- Capable of facilitating service coordination, referring clients, and negotiating service delivery on behalf of carers.
- Ability to work both independently and as part of a collaborative team.
- Excellent problem-solving skills with a strong understanding of the community care sector and the challenges faced by carers of individuals with disabilities, chronic illness, mental health conditions, or the elderly.
- Current unrestricted Australian Drivers Licence and willingness to undertake travel in accordance with responsibilities of the role.
- Current First Aid and CPR Certificates
