LiveBetter has an exciting opportunity for a Full Time Carer Support Planner to join our Carer Gateway team. This position is a fixed term until 30.06.2027 that will commence as soon as practicable and can be based out of Orange, Nyngan or Griffith.

 

About the Role

As a Carer Support Planner, you will support carers to assess their needs and develop personalized support plans using tools like the Carers Star™. You will assist carers in identifying their strengths, informal networks, and community resources to help improve their well-being. Carer Support Planner also coordinate access to services, including emergency respite care, and help carers navigate external support systems such as My Aged Care or the NDIS. Additionally, they monitor and evaluate the effectiveness of support provided and collaborate with both internal and external teams to ensure carers' needs are met.

 

About You

 

The successful candidate will possess:

  • Ability to pass a National Criminal History Check
  • Background in Community Services, Health, Welfare, Social Services, or Education, or a minimum of three years’ relevant industry experience.
  • Ability to conduct complex telephone assessments, gather accurate information, and prioritize actions effectively.
  • Strong interpersonal skills with the ability to provide superior customer service and communicate effectively with diverse stakeholders.
  • Capable of facilitating service coordination, referring clients, and negotiating service delivery on behalf of carers.
  • Ability to work both independently and as part of a collaborative team.
  • Excellent problem-solving skills with a strong understanding of the community care sector and the challenges faced by carers of individuals with disabilities, chronic illness, mental health conditions, or the elderly.
  • Current unrestricted Australian Drivers Licence and willingness to undertake travel in accordance with responsibilities of the role.
  • Current First Aid and CPR Certificates
