We're looking for an experienced and passionate Senior Care Partner to lead the delivery of our Community Programs and Respite Services across Albury and the Riverina.

About the Opportunity
As our Senior Care Partner, you'll play a key leadership role in overseeing customer engagement, care coordination, community programs and respite services across a diverse and growing portfolio, including our well-regarded Anson Cottage Respite Service.

You'll work closely with customers, families, community stakeholders and internal teams to ensure services are responsive, person-centred and focused on achieving meaningful outcomes.

 

Your responsibilities will include:

  • Managing My Aged Care referrals and customer onboarding
  • Coordinating assessments, care planning and ongoing reviews
  • Leading community programs and respite service delivery
  • Overseeing rostering, service coordination and daily operations
  • Building strong partnerships across local communities
  • Identifying opportunities for service growth and innovation
  • Maintaining compliance with aged care standards and funding requirements
  • Driving continuous improvement and customer-focused outcomes

 

About You

You'll thrive in this role if you have:

  • Experience in aged care, community services, care coordination or a related field
  • Strong organisational and problem-solving skills

  • Experience coordinating services and supporting customer outcomes

  • Excellent communication and stakeholder engagement skills
